Some sources advise limiting goitrogenic foods – cruciferous vegetables like broccoli, kale, and cabbage, as well as soy and millet – because goitrogens can interfere with iodine uptake in the thyroid. However, cooking reduces goitrogenic activity substantially, and these are among the most nutrient-rich foods available. Blanket avoidance means giving up significant nutritional value for a risk that is largely theoretical at normal intake levels. For patients who eat a lot of raw cruciferous vegetables or soy, we focus on making sure dietary iodine is adequate – through iodized salt, seafood, or dairy – and on timing thyroid medication appropriately away from soy consumption, which can affect levothyroxine absorption. The goal is to keep these foods in the diet, not remove them.

Should I take thyroid medication away from certain foods?

High-fiber foods, calcium, iron, and coffee can all reduce absorption if taken too close to the medication. Soy can interfere with levothyroxine absorption and should be timed away from the dose.
