Structured care plans include scheduled follow-up sessions designed to support steady progress and continuity of care. Packages must be used within 12 months of purchase.

1. $750 for 6 follow ups ($125/appointment)
2. $1200 for 12 follow ups ($100/appointment)

Payment is taken at the end of your appointment if paying out of pocket. 

If there is a suspected copay, we will wait until the claim comes back from insurance with the amount.

Typically there will be no copay because we bill using preventative coverage.

No, all labs are out of pocket, but can be paid for with an HSA/FSA card.

Initial Assessment and Follow-up Appointments (for lab review) are not included in the cost of the lab test, but can be covered by insurance. 

A superbill is a detailed invoice that includes diagnostic and treatment codes, allowing clients to submit it to their insurance for potential reimbursement. This can only be done for out of network plans.

How do I know if I will be reimbursed? How much will I be reimbursed?

This depends on if you have out of network coverage for nutrition counseling; you will need to call your insurance company to ask.

We tend to see reimbursement of about 50% of our cash rates.

If desired, we can make recommendations for supplements or additional lab testing. While this is not a requirement of working with our team, it can be supportive to your health journey.

Supplements are not required, but it is often part of our recommendations. Please communicate with your FNS provider why you don’t want to take supplements, so we can provide the best alternative recommendations for you.
