How it Works

Complete Your Application
It’s just a few key questions to help us assign you to the provider who is the best fit for your needs! After you complete the form, an FNS team member will follow up within a business day to give you access to our patient portal and guide you through the next steps (including your insurance eligibility check).

Initial Comprehensive Assessment
You’ll meet with your assigned dietitian or nutritionist for a comprehensive 70-minute session, during which we will review your health journey, medical/nutrition history, symptoms, current diet & lifestyle, goals, and motivators. By the end of your first appointment, we’ll start building a healing plan that is tailored to your unique needs.

Personalized Plan
You’ll leave your first session with a clear path forward, including 1-3 specific nutrition and lifestyle changes you can start incorporating right away to support your well-being. Our goal is to take the stress and guesswork out of nutrition, helping you build lasting habits that fit into your unique life. In subsequent appointments, we'll check in on your progress and readjust your plan, as needed.
